It is FutureCare’s admissions practice to offer all residents and/or their authorized representatives the opportunity to enter into a binding arbitration agreement as an alternative form of dispute resolution. Arbitration Agreements are presented to all residents and/or their authorized representatives upon admission along with their admissions agreements. Services at FutureCare are NOT conditioned upon a resident and/or their authorized representative agreeing to arbitration. If you would like to obtain a copy of FutureCare’s arbitration agreement or, if authorized, a copy of a specific binding arbitration agreement on behalf of a resident, please contact the administrator.
