Our nurse practitioner works closely with your physician, along with nurses and therapists to create an individualized care planned based on your personal healthcare needs. A LifeBridge Health doctor of physical medicine and rehabilitation (physiatrist) is also part of your treatment team. FutureCare Courtland features Renalis Kidney Services offering comfortable, in-patient dialysis services to our patients who need it.
FutureCare Courtland holds the prestigious National Heart Failure Certification from the American Heart Association, signifying our expertise and commitment to providing comprehensive care specifically within the skilled nursing environment, catering to the complex needs of heart failure patients.

This place is very clean, the staff is friendly and I felt welcomed as soon as I walked in the door.
Very Happy with the care i received! Everyone from my nurses to my Physical Therapist/ Occupational Therapist to my Social Worker was aces ! They truly care about your wellbeing. Couldn’t ask for a better team ! Went in unable to walk , came out 75lbs lighter , and healthier then ever before . So happy & blessed !
