Four FutureCare Facilities Receive National Award for Low Hospital Readmission

  

Baltimore, MD--March 26, 2014--FutureCare Irvington, FutureCare North Point, FutureCare Old Court, and FutureCare Pineview all received the Providigm Embracing Quality Award for having a low rate of readmission to hospitals. The facilities received these awards during AHCA/NCAL's annual Quality Symposium in February.

Providigm is a quality management system used by long term care facilities to track and improve the quality of care and life of their residents. The winners of the Embracing Quality Awards are regarded as the top-performing long term care facilities in the U.S. and Canada.

While presenting the awards, Providigm's Accreditation Program Director Barbara Baylis stated that "the facilities announced tonight will undoubtedly serve as role models for their peers in long term health care."
