Thank you for your interest in FutureCare Homewood. We take a lot of pride in the full range of post-acute care services we offer our residents. At Homewood, we utilize a resident centered plan of care to help our residents achieve or maintain his or her highest practicable physical, mental, and psychosocial well-being. We work with our residents and their family members to develop a personalized plan that promotes our residents individual goals and preferences.
Some of the programs we offer our residents include three of FutureCare's proprietary programs: HeartStrong, P2CU, and VitalStrong.
HeartStrong- Is our Cardiac Rehab Program overseen by a Cardiologist. The 39 bed Cardiac Rehab Rehabilitation program in collaboration with Union Memorial Hospital cardiology group, offers our residents a weekly assessment as well as an individualized education plan by our in-house nurse navigator who continues to follow your loved one's progress 30 days after discharge to prevent re-hospitalization.
P2CU-Is our Progressive Pulmonary Care Unit overseen by a Pulmonologist and Critical Care Specialist. Our 31 bed P2CU is staffed with highly qualified respiratory therapists 24 hours a day, providing ventilator care, tracheostomy care, and an ongoing successful weaning program to our residents under the close weekly supervision of our pulmonologist and medical director.
VitalStrong- Is our State of the art Rehabilitation Gym overseen by a Physiatrist. Our Rehabilitation gym, renovated in 2016, offers state-of-the-art equipment and the latest in treatments.
